Please fill out this survey for Serve Day so that the Serve Day committee knows how much food to prepare for the day. All meals are covered for Serve Day team leaders and committee members. Extra meals may be purchased for family members on Serve Day.
If you would like to purchase a lunch for Serve Day, please fill out this survey. Breakfast and dinner are provided for all employees, and lunch can be purchased for an extra fee. If you are a team leader or committee member, your lunch is complimentary and you do not need to fill out this form.
