New tax benefit forms being distributed

Wednesday, March 30th, 2016

If you were offered health coverage in 2015, you will be receiving a new tax form this week. The 2015 1095-C will be delivered to campus boxes today (March 31). This tax form includes information about the health insurance coverage offered to you and, if you were enrolled in the Pioneer Regence Medical Plan in 2015, your family as well.

Please review your 1095-C and let us know if there is any incorrect information. As part of the new Health Care Reform and the Affordable Care Act (ACA) requirements, George Fox will report this information to the IRS as well.

In most cases, you do not need to wait for the Form 1095-C in order to file your 2015 U.S. Individual Income Tax Return (IRS Form 1040, 1040A, or 1040EZ). You may receive multiple Form 1095-Cs if you worked for multiple employers in the previous calendar year.

If you would like to request that the form to be mailed to you, please email your request to Darby Thiessen in Human Resources at dthiessen@georgefox.edu.
