Providence members: Take advantage of healthy eating and active lifestyle program

As a Providence Health Plan member, you may be eligible to participate in the Healthy Eating and Active Lifestyle (H.E.A.L.) program, a free lifestyle coaching service.

H.E.A.L. is an evidence-based weight management program aimed at helping you build healthy habits, improve your well-being, and lower your risk for developing Type 2 diabetes. The Providence live virtual one-on-one and group classes allow you to interact with your coaches and other program participants. You will also have opportunities to interact one-on-one with your coach.

To participate, you cannot have a previous diagnosis of Type 1 or 2 diabetes and must be:

  • 18 years or older
  • Have a BMI of 25 or higher, or 23 for Asian Americans
  • Find out your BMI (click here)
  • Have at least one of the following:
  • Fasting blood sugar of 100-125 mg/dL (110-125mg/dL for Medicare members)*
  • Hemoglobin A1c of 5.7%-6.4%*
  • History of gestational diabetes*
  • Diabetes risk score of 5 or higher (Risk Test: click here)*
  • * If you are not eligible for the H.E.A.L. program, you can still enroll into the Providence standard health coaching program.
