Take survey to earn chapel credit, be entered to win Amazon gift card
Monday you received a second invitation to participate in the National College Health Assessment (a survey we use every three to four years to assess student health behaviors in order to provide better services). It was sponsored and distributed by the American College Health Association (ACHA).
As an incentive to participate,
• All students completing the survey will be credited for one chapel credit
• 25 students will be randomly selected by ACHA to receive $10 Amazon gift cards
