The form is listed on the finance page <a href=\"https:\/\/www.georgefox.edu\/offices\/fin_affairs\/new_vendor_account_setup_form_04-12-221.pdf\"><strong>here<\/strong><\/a>.<\/p>\n<p>When communicating with your vendors, please have them submit these forms, along with their W-9 and banking verification documents, to Frank Charles, our purchasing director, at <a href=\"mailto:fcharles@georgefox.edu\" target=\"_blank\" rel=\"noopener\">fcharles@georgefox.edu<\/a>.<\/p>\n<p>As a reminder, no purchases can be made without an approved George Fox PO number.
